Accident Investigation 

The Health and Safety at Work etc Act 1974 states that there is a legal duty for specified injuries, diseases and dangerous occurrences to be reported to the appropriate enforcing authority.

As an employer, a person who is self-employed, or someone in control of work premises, you have legal duties under RIDDOR that require you to report and record some work-related accidents by the quickest means possible.

Reason for reporting:

  • To investigate the cause of the incident
  • To prevent a reoccurrence of the incident
  • To enable duty holders to comply with health and safety legislation

The Health and Safety Team will investigate reported incidents depending on nature and extent of the incident, previous history of incidents at that premises, and if the incident has a wider, national dimension.

What should be notified?

  • All fatal injuries
  • All major injuries, which are:
    - fracture
    - amputation
    - dislocation
    - loss of sight
    - chemical or hot metal burn to the eye
    - electrical shock or electrical burn
    - an injury leading to hypothermia or requiring resuscitation or requiring admission to hospital
    - inhalation, ingestion or absorption of substances
    - acute illness resulting from exposure to biological agents or toxins
  • Over 3 day injuries:
    - where an employee or self employed person has an accident and the person is away from work or unable to work normally for more than 3 days
    - injuries to members of the public where they are taken to hospital
    - work related diseases
    - dangerous occurrences, where something happens that does not result in a reportable injury but which could have done

  • The self employed
  • A person in control of premises
