Who the service is for
The service is for older people who have either been in hospital or who need support to enable them to remain at home.

The service provided
This is a ‘floating’ support service, which provides support to people in their own homes.  The type and level of support provided is dependent on the needs of the service user.

Location
Ammanford and the Tywi Valley.

Type of Accommodation
The service can be provided to people who own their homes or live in rented accommodation. 

When the service is available
Support is provided during normal office hours, with some flexibility to meet the needs of the service user.   An out-of-hours on-call service is available for emergencies that arise outside of normal office hours.  Each service user receives up to four hours of support a week.

How long the service can be used for
People can use the service for up to two years.

How to access the service
Council services and voluntary and private service providers can make referrals including Social Workers and other Health Care professionals. People who want to use the service can also refer themselves, but the service provider will need to consult other professionals involved in providing a service to the possible service user.  The service provider will also assess the service user’s needs before that person can be accepted onto the scheme.

Cost
The service user will not need to pay for the support they receive.  This service is funded through the Supporting People Grant paid direct to the service provider by Carmarthenshire County Council.
