The Monthly Monitoring System is used to collect vital information about the performance of housing related support services in Carmarthenshire. This information:

 is collated and is reported quarterly to the Supporting People Planning Group.
 informs monitoring visits that are undertaken
 also contributes to regular service reviews.

This new system will also enable us to start looking at the outcomes of support for service users. The system includes our new exit questionnaire which will help us establish what service users have got out of their support.
